Senior Care

The Community Health Choices (CHC)

The Community HealthChoices (CHC) Waiver Program is Pennsylvania’s long-term care waiver for the delivery of both physical health and long-term health care services through a per-patient managed care system put in place by the Pennsylvania Department of Human Services (DHS) and Aging. The goal of this long-term care waiver is to improve coordination of physical and behavioral health systems, as well as Long-Term Services and Supports (LTSS).

Eligibility is based on a level of care assessment, a financial eligibility determination and program eligibility.

  • Eligibility Criteria: Individuals must be 21 years or older and eligible for Medicaid or Medicare.

  • Services Covered: Long-term services and supports, nursing facility services, and home and community-based services.

    The Attendant Care Waiver / Act 150

ACT 150 provides supports and services so people with physical disabilities can continue to live at home. The state-funded services are offered for a fee to persons who do not qualify financially for Medicaid. Eligibility is based on a level of care assessment, a financial eligibility determination and program eligibility. This waiver provides personal care and support services to physically disabled individuals aged 18-59 to help them live independently

  • Eligibility Criteria: Must be 18-59 years old, physically disabled, and meet financial criteria.

  • Services Covered: Personal care, home health services, assistive technology, and home modifications.


INTELLECTUAL DEVELOPMENTAL DISABILITY SERVICES

OBRA Waiver

The OBRA Waiver is a Medicaid Home and Community Based program in Pennsylvania that supports individuals with severe developmental physical disabilities to live independently and compensates unpaid family caregivers through a self-directed option. OBRA stands for the Omnibus Budget Reconciliation Act, also known as the Nursing Home Reform Act of 1987, which set federal standards for levels of care toward residents. Eligibility is based on a level of care assessment, a financial eligibility determination and program eligibility. The OBRA Waiver is designed for individuals with developmental disabilities that occurred before age 22 and who need assistance to live independently.

  • Eligibility Criteria: Must have a developmental disability that occurred before age 22 and meet financial criteria.

  • Services Covered: Personal care, home health services, behavioral therapy, and home modifications.


Community Living Waiver
The Community Living Waiver is a Medicaid waiver that is available statewide in Pennsylvania. The  Community Living Waiver is a Medicaid program designed to support individuals with intellectual disabilities or autism and children with developmental disabilities. It aims to facilitate living in community-integrated settings rather than institutionalized environments. This program emphasizes deinstitutionalization and provides an array of services to promote independence and integration into the community. It includes participant-directed and agency-based service models catering to a broad range of needs and preferences. Unforgettable Caregivers provides the following services:

P/FDS waiver
The P/FDS waiver is a home and community-based waiver program that helps individuals with an intellectual, developmental, or autism disability to live more independently in their homes and communities. The P/FDS waiver provides various services that promote community living, including self-directed and agency-based service models1. It is designed to supplement, not replace, the supports and services available to individuals and their families through other sources, such as the base- funding system.
