Service Description

During Intake, A case manager will create with you a plan that is best for your health needs. An assessment will be performed and your case manager will explain all the details of your plan of care. Both you and your case manager will review to be sure you understand and are comfortable with the services that will be provided. Prior to the commencement of services, you will be provided with an information packet. The information provided will be delivered in a form that is easily read and understood. The following is a check list of all information that will be given, o A listing of the available home care services that will be provided o The hours when those services will be provided. o Fees and total costs for those services on an hourly or weekly basis. Our team will answer any questions you or your caregivers have and demonstrate any techniques necessary for your care. Depending on your needs, your first visit may last up to three hours.
